Is it illegal to terminate an individual based on false information?

No....Theres a paragraph where you sign your name which states that all the information you provided is true to your knowledge. Is it illegal to terminate an individual based on false information?
Depends largely on whether you live in an at-will employment state. Is it illegal to terminate an individual based on false information?
It depends on where you live. In some states the company does not need any reason at all to terminate an individual.
It depends where you live, each state has its own laws.

In NY state in the US, an employer ca terminate you for any reason, or for none. Literally, they can terminate you if they don't like your haircut, with no prior notice and without the haircut being part of the job description.
You can be fired for anything or nothing in most cases, sorry toots. You can also quit for any or no reason. Let freedom ring.
